IMPORTANT
Students are not allowed to attend class until the school has received all the required documents and proof that your child has received all currently due immunizations.
Starting January 1, 2016: Parents or guardians of students in any school or child-care facility, whether public or private, will no longer be allowed to submit a personal beliefs exemption to a currently-required vaccine.

All students must be enrolled by a parent or legal guardian and provide the documents outlined below:
All students (Pre K-12)

1. Birth Certificate

2. Proof of Residence
*Must be in a parent/guardian's name* Examples: Recent utility bill, rental agreement, property tax payment receipt, mortgage documents, or military housing orders.

3. Immunization Record
|
5 DTaP (4 doses meet requirement if at least one was given on or after the 4th birthday; or 3 doses meet requirement if one given after age 7)
|
4 Polio (3 doses meet requirement if at least one was given on or after the 4th birthday)

3 Hepatitis B
|
2 MMR (both doses on or after 1st birthday)
|
2 Varicella (2 doses now required for new students entering elementary school as of 7/1/2019)

UTK: Universal Transitional Kindergarten
Available for any students who turn four years old on or before Sept. 1, 2022
The same immunizations requirements apply to enroll in the UTK program.
Kindergarten
Children must turn five (5) years old on or before Sep. 1, 2022, in order to be eligible to attend kindergarten for the 2022-23 school year.
All Grade 1 students
Must be at least 6 years old by Sept. 2nd of current school year.
